Welcome to the Faith Formation Registration area. Please review the family letter(s) appropriate to the age of your children and the calendar. Forms required for Faith Formation are available below.

(If you are a returning family, watch your mail for a pre-filled registration form to approve and send back for easy registration.)

Elementary and Middle School Family Letter

After you have completed the fillable PDFs below, please save a copy of the document, then click the email button on the bottom of the page, and then when the email to Michelle pops up, attached the saved copy and click “Send”. Thank you.

Form A

Faith Formation Registration — Required for all new families or returning Faith Formation families who wish to submit a new form. This also includes Sacramental Preparation Information for the Faith Formation Families. Save and email a copy to Michelle.

Form B

Parental Authorization Form — Required for all families. Save and email a copy to Michelle.

Volunteer Opportunities Save and email a copy to Michelle.

Sacramental Preparation Registration for Newman Catholic Students. Save and email a copy to Michelle.

Tuition Payment Information

Faith Formation – $80 per student ($80 initial payment), $150 maximum per family. Sacramental fees are included within Faith Formation tuition.

The Sacramental Fee for Newman Catholic School is $30.

If you need confidential tuition assistance or an alternative payment plan, please contact the Faith Formation Office at 715-842-1810.
